• I Agree and affirm that she want to get the piercing. I further affirm that she doesn't have a medical or skin condition that may interfere with the procedure or healing of the piercing

• I Agree that both Artist and Studio have given me the full opportunity to ask any and all questions about the piercing procedure and have answered these questions to my total satisfaction. I have been fully informed of the risks of getting a piercing. I fully understand that these risks, known and unknown can lead to infection, scarring.

I Agree to everything written above and by signing this consent form, I hereby release AYEPIERCING it's employees from all manners of liabilities, claims, actions and demands in law or in equity
